ENROLLMENT PROCEDURES FOR STUDENTS NEW TO THE ALAMEDA UNIFIED SCHOOL DISTRICT

Enrollment for the 2009-2010 school-year will be by appointment only starting April 14th At your appointment, you will need:
1. Two Different, Current Utility Bills – Electric/gas/garbage/water – (must have parent’s name and living address). We do not accept bank statements, phone/cell bills, cable bills.
2. One of the following:
Property Tax Bill with parent’s name and property address Or
Rental/Lease Agreement with the parent’s name, student’s name and address, as well as manager or owner’s name and phone number
3. Parent/Guardian Picture ID (Current CA Driver’s License, Current CA Identification Card, Military ID, Passport)
4. Immunization Records
| · -Polio: 4 doses, but 3 will meet the requirement if at least one was given on or after the 4th birthday | · -DPT: age 7 or older, at least 3 doses · -Hepatitis B: 3 doses (Kdg 7th) |
| · -MMR: 1 dose (if given after 1st birthday, 2nd dose is required for Kdg. 7th) | · -Varicella (Chickenpox) |
5. Birth Certificate (Passport not accepted)
6. Most Current Report Card (9th grade students only) or Transcript
7. Withdrawal/Clearance Papers (Mid-Year Transfers Only)
8. Disciplinary Records (Mid-Year Transfers Only)
9. Attendance Record (Mid-Year Transfers Only)
10. Social Security Card (If Available)
11. Proof of Guardianship (If Applicable)
12. Current IEP or 504 plan (If applicable)
**A PARENT OR LEGAL GUARDIAN MUST BE PRESENT**
**All enrollment forms must be filled out at the time of the appointment**
Family Student Affidavits, Interdistrict and Intradistrict Permits can be obtained from Student Services located at 2200 Central Avenue, Suite B, (510) 337-7072 and require Student Services approval. Revised AHS 12/08
