Athletic Eligibility and Forms

Physical Eligibility Each form in the section below must be completely filled out and turned into the Athletic Office before your son/daughter is able to tryout for or participate on any athletic team.

Academic Eligibility We expect our student-athletes to be students first and maintain exemplary character in the classroom by being in class on time everyday, modeling mature behavior and focusing on academics. If any student-athlete falls below a 2.0 he/she must see our Athletic Director for the possibility of being granted a one-time probation (only if GPA is between 1.50-1.99). If probation is granted, there will be weekly progress checks in order to maintain eligibility.

Transfer Eligibility If your son/daughter has NOT attended Alameda High School for 12 months, he/she will need to see the Athletic Director for transfer paperwork. Transfer students are NOT allowed to participate in ANY game competition until cleared. Participation prior to clearance could result in forfeiture of games and ineligibility.

Please note:
*The entire packet must be signed and initialed by the same parent.  (Should the packet be returned for missing signatures or initials it must be signed/initialed by the parent who originally signed it.) A packet that is returned for a signature/initial will not be accepted on the same day it is returned.


**Forgeries have started to become an issue more and more.  Any packets containing forgeries (whether parent gives permission for student-athlete to sign their name or not) will be returned for parent to sign and the student-athlete will be sat out for 5 days before they can participate.
